  • We support and care for adults living with a learning disability, who may also have a diagnosis of autism, in specialist supported living, residential services and hospitals. Our aim is to deliver effective and outcome based services within person centred, professional care environments, supporting people to have the most choice and independence possible. We set high standards for ourselves in the delivery of care, and most importantly, we put the people we support at the heart of everything we do.

    Within your role you will undertake work that is meaningful to the people we support in our services and socially important within the local community. You will be responsible for the preparation and provision of dietary meals for those in our care within the guidelines of care plans or service requirements.   • Based in Hartlepool, Hope House is a care home with nursing, which delivers 24 hour care and support for up to 11 people with learning disabilities and complex needs in self-contained bespoke apartments, adapted to meet their individual needs. The name Hope House was chosen by one of the gentlemen who moved into the service when it first opened. He said, “I am always hoping for something and always hoped to live in this area.”

     

    Hope House was specially designed in partnership with local commissioners and community teams to create individual, bespoke, self-contained apartments that provide personalised support for people. The service design is split into two separate elements, based on the specific individual requirements of the people we support.
